Transition Support
Specialist support across major transitions — primary to secondary, KS4 to post-16, and into adulthood.
Who it's for
Young people approaching a significant educational or life transition where regression risk is high.
How it works
Transition is where progress is most often lost. We plan transitions over months, not weeks: visiting future settings, building familiarity, rehearsing routines and supporting the receiving team to hold the work we have done.
What the placement looks like

Map
Transition mapped backwards from the move date — usually 3 to 6 months out.

Familiarise
Visits, photo books and rehearsal of routines with the receiving setting.

Hand over
Receiving team briefed on profile, triggers and what works.
